Lafayette Hunters Plaza Building 3218 Daugherty Dr Lafayette, Indiana 47909 WHO WE ARE LOOKING FOR: Franciscan Physician Network is seeking an Advanced Practice Provider who specializes in Occupational Medicine to join our team at our Franciscan WorkingWell Clinic in Lafayette, Indiana. WHAT WE NEED: Candidate will have ACLS/BLS certifications and will be able to see all patient types. Occupational Medicine experience of 2 years is preferred, but not required. Candidate should be comfortable with workman’s compensation injuries, Department of Transportation physicals, musculoskeletal injuries, and other work-related injuries or services. WHAT YOU CAN EXPECT: Practice Hours: Monday – Friday, 7:00 a.m. – 7:00 p.m. Flexible provider schedule; No weekends or holidays Provide virtual support to outreach locations in Western and Central Indiana Average outpatient volume: 17 - 22 patients per day Telehealth offered On-site training required in Indianapolis for up to 6 weeks; Mileage will be reimbursed Common Diagnoses: Exposures Lacerations Musculoskeletal injuries Pre-employment, DOT, and Non-DOT physicals Sprain and Strains WHO WE ARE: Franciscan Health is a non-profit health care ministry with primary and specialty care physician groups located throughout Indiana and Illinois.
