WHAT YOU CAN EXPECT: Collaborate with multidisciplinary teams to evaluate and assess patient's medical and physical needs. Refer patients to appropriate services if patients are identified with depression, anxiety, grief or mental illness. Utilize consultation data and social work experience to plan and coordinate client or patient care and rehabilitation, following through to ensure service efficacy. Assist patients and their caregivers in identifying goals of care and assist with unmet needs through linkages to community resources.
