POSITION SUMMARY: Perform functions of a Certified Pharmacy Technician interacting with partnered members and care givers via telephonic visits collecting medication reconciliations, enrolling patients telephonically, and providing overall support for the Med Management business. Collaborate closely with members, pharmacists, and physicians to increase the members understanding of current medications and compliance working within the limits of established policy. Perform duties to prepare and dispense prescription medications and supplies under the direction of a Registered Pharmacist, in a fast-paced closed-door Pharmacy where attention to accuracy, regulatory compliance, and efficiency are critical to patient safety and care, working within the limits of standard or accepted practice.
ESSENTIAL FUNCTIONS:
- Communicate the details of CHealthcareRx program features, objectives, and benefits to members and/or caregivers telephonically or in-person in hospital setting.
- Complete patient health questionnaires and an accurate medication reconciliation to include prescription and over the counter medications during in-home visits.
- Develop rapport with members and/or caregivers to gain confidence and trust.
- Collaborate with the consultant pharmacist as well as member’s physicians to increase the members understanding of current medications and compliance.
- Perform job assignments in full compliance and complete understanding of the State and Federal laws and internal policies and procedures governing the handling, dispensing and distribution of controlled substances.
- Review and interpret physician medication orders to ensure appropriate therapies and enter prescription data to computer system to process new prescription orders and reassignments in preparation for Pharmacist verification. Select drugs from stock, count and package as required, and accurately label prescriptions for Pharmacist verification.
- Responsible for reporting daily productivity metrics related to role.
- Escalate concerns regarding the patient’s medications, health and well-being to the proper teams.
- Accountable for following department guidelines, company policies, and HIPAA privacy rules.
- Provide excellent customer service to members, caregivers and partners.
- Perform other related tasks as assigned.

- Ability for overnight travel 10% and occasionally lift up to 20 pounds.
Qualifications
- High School diploma or equivalent.
- Current and in good standing Pharmacy Technician Certification for assigned jurisdiction. National Certification (PTCB) preferred.
- Two (2) years of pharmacy tech or nursing assistant experience strongly preferred.
- Previous experience in sales or customer service highly preferred.
- Reliable transportation, valid driver’s license in good standing and proof of car insurance.
