Community Care Cooperative (C3) is a 501(c)(3) non-profit, Accountable Care Organization (ACO) governed by Federally Qualified Health Centers (FQHCs). We are a fast-growing organization founded in 2016 with 9 health centers and now serving hundreds of thousands of beneficiaries who receive primary care at health centers and independent practices across Massachusetts. Job Summary:
This position is situated within the Health Equity & Racial Justice program. The Program Manager will play a key role in assisting the Manager in developing and maintaining our ACO capabilities for delivering Culturally and Linguistically Appropriate Services (CLAS) to our members. In addition, the Program Manager will support the Director and Manager with the execution of risk contracts, ensuring our compliance with reporting requirements. Furthermore, the Program Manager may provide support to the Manager in collaborating with FQHCs to address inequities in health outcomes within the diverse demographic populations of our members. This includes specific support for our collaboration with acute hospitals and our FQHCs.
Responsibilities:
- Support the execution for components of the MassHealth and other risk contracts and initiatives
- Provide direct support to the Manager and/or Director of Health Equity and Racial Justice for special projects and initiatives
- Provide support and coordination for assigned activities, ensuring assignments and projects are delivered on-time and within scope
- Use critical thinking skills to assist in the definition of project scope and objectives, involving all relevant stakeholders and setting achievable timelines with appropriate interim milestones to mark progress
- Assist in driving the objectives of the select projects and committee(s) forward by collaborating and serving the team’s needs; oversees meeting logistics by working with leadership to determine meeting agendas and document key takeaways
- Work collaboratively across teams, within the multi-matrixed nature of work environment, to facilitate meeting discussions, engage all relevant stakeholders, and develop useful presentation materials with the goal of advancing and achieving contract deliverables
- Collect and disseminate accurate, relevant and timely information to the members of the select project team or committee
- Develop a detailed project plan to monitor and track progress; manage changes to project scope and schedule; identify, track, and develop mitigation plans for risks; reports and escalate as needed
- At times, may need to support the Manager with managing external consultants and contractors, helping them adapt to the C3 culture and obtain information/data necessary to successfully complete their assignments
- Develop expertise in the technical specifications of the reporting requirements of risk contracts and serve as the Subject Matter Expert (SME) in guiding internal and external stakeholders
- Support the Director and/or Manager to conduct organizational assessments and develop comprehensive reports
- Other projects and duties as assigned
Required Skills:
- 3-5 years’ experience in population health or public health preferred
- Strong technical writing and editing skills
- Proficiency in MS Excel and PowerPoint
- Experience in document-management, with aptness for managing a detailed tracking system
- Great integrity and produces accurate work with close attention to detail, especially in the completion of final deliverables to internal and external stakeholders
- Experience in Health Equity, Diversity, and Inclusion
Desired Other Skills:
- Familiarity with the MassHealth ACO program
- Familiarity with Federally Qualified Health Centers
- Ability to work independently and collaboratively with healthcare professionals
- Experience in professional communication and public speaking
- Experience working in community/healthcare setting
- Familiarity with population health informational technology platform
- Lived experience with racism/commitment to racial justice is highly preferred
Qualifications:
- Bachelor’s Degree or appropriate experience in lieu of education
- Masters’s Degree in public health or related field preferred
