Under the supervision of the Chief Behavioral Health Officer or their designee, the Behavioral Health Nurse Clinical Coordinator plays a pivotal role in the integration, implementation, and coordination of mental health, substance use, and primary care services within the health clinic. The coordinator is responsible for providing clinical and operational leadership, ensuring high-quality, patient-centered care, and facilitating interdisciplinary collaboration among behavioral health providers, medical teams, and support staff. The position emphasizes holistic care, addressing the physical, mental, and social health needs of patients, with a focus on accessibility, equity, and culturally competent care. The coordinator will collaborate with a multidisciplinary team to enhance patient care, optimize resource utilization, and ensure regulatory compliance. The Nurse Clinical Coordinator may have both clinical and administrative responsibilities. This work is primarily in an outpatient setting.
1. RN / LPN licensure that is current, valid, and in good standing in the state of North Carolina (or Nurse Licensure Compact State).
2. Minimum of 1 year in a behavioral health, psychiatric, or substance use treatment setting with a minimum of 1 year in clinical coordination, population health, or care management is highly preferred.
3. Knowledgeable in assessment, modalities (i.e. motivational interviewing, cognitive behavioral therapy, solution focused therapy, psychodynamic psychotherapy, and others), care planning, care coordination, utilization management and working with comorbid populations (chronic health conditions, mental health, and substance use).
4. Adequate knowledge or experience in the use of common screening tools, motivational interviewing, cognitive behavioral therapy, trauma informed care and other evidence and strengths-based interventions.
5. Knowledge of and confidence in assessing and evaluating physical, mental health and substance use conditions.
6. Adequate knowledge of pharmacology, in particular psychopharmacology and other behavioral health and substance use treatments and interventions, and counseling and educating patients about these treatments.
7. Ability to interact and render service professionally to diverse populations, including pediatrics, geriatrics, and special populations.
8. Interacts with respect and in a compassionate manner with patients, staff, external customers and maintains confidentiality and professional boundaries.
9. Proficiency with electronic health records and other healthcare related technology.
10. Excellent communication, interpersonal, organizational, and team-based skills.
11. High degree of emotional intelligence and ability to engage in critical thinking.
12. Abilify to demonstrate flexibility and adaptability in a fast paced, dynamic, and diverse setting.
13. Must be able to travel / commute independently between CRCHC sites.
- Spanish language proficiency is highly preferred.
- Certification in Psychiatric-Mental Health Nursing (PMHN) is preferred (RN only)
Key Responsibilities
Collaborate with psychiatric providers, primary care providers, therapists, other health care professionals and paraprofessionals to inform, develop, and implement individualized treatment or care plans. Assists with patient health education and follow up on established patient plan(s) of care.Coordinate care for patients with complex mental health, substance use, and medical conditions, ensuring seamless integration throughout multidisciplinary team.Monitor and facilitate patient referrals, follow up appointments, and transitions among internal and external care providers and agencies.Responds to patients questions / doctors office requests in a timely manner.Conduct comprehensive behavioral health and substance use screenings and assessments to assist in providing and planning care.Support patients and caregivers who may be in distress and require de-escalation, crisis disposition, or other community resources, as needed.Monitor patient outcomes, treatment efficacy, and progress, and processes to identify areas for improvement.Perform clinical skills such as sterile dressings, EKGs, pulmonary treatments and the safe and accurate administration of medications and immunizations.Assist medical and behavioral health staff with routine clinic tasks, examinations, and treatments.Performs and assists with the triage of patients on-site and via telephonic media for appropriateness and timeliness of care per approved triage protocols and standing orders. Promptly and accurately document these in the triage log and electronic health record.Perform clinical skills such as sterile dressings, EKGs, pulmonary treatments and the safe and accurate administration of medications and immunizations. Maintains PPE Inventory / log and medication inventory.Lead and participate in quality improvement initiatives to enhance patient experience and promote best practices of integrated care.Ensure compliance with regulatory standards, clinical protocols, and nursing process.Act as a liaison between behavioral health and primary careCoordinates the huddle sheet and participates in or performs daily, as assigned.Provide staff training, education, and development regarding behavioral health and physical health topics (i.e. chronic health conditions, medication adherence, motivational interviewing, trauma informed care, culturally competent care, and special population care).Assist in the development and implementation of clinical protocols that promote integrated behavioral and physical health care.Follows and maintains strict adherence to all established Infection Control guidelines and OSHA standards established by the organization.Maintain patient confidentiality and protect operations by keeping protected health information confidential and secure.Demonstrates the ability to effectively organize and prioritize work and utilize available resources to assist with the efficient flow in the clinicParticipates in mandatory in-services and drills, attends staff meetings and other training, including appropriate age specific educational training offered on regular basis for professional development.Assists with the orientation and education of new clinical staff.Participate in interdisciplinary meetings, case reviews, and services planning sessions, as assigned.Document care in the patient health records, maintain care registries and databases, and prepare reports and presentations.Report to the Chief Behavioral Health Officer and the Behavioral Health Program Manager regarding patient care and clinical services and operations.Promptly informs DOCS of actual or potential quality of care excursions.Performs all other duties as assigned.
