Summary of Position
The Licensed Clinical Social Worker Clinician (LCSW) provides direct Licensed Clinical Social Worker services to clinic patients to improve and promote overall health, with a focus on addressing the behaviors that impede optimum health. This role coordinates services with the interdisciplinary team to provide consultation regarding mental health and/or substance use/abuse counseling while facilitating the development of a comprehensive plan of care to address the whole person. The LCSW adheres to all internal policies, widely accepted professional practice standards, and requirements for professional practice and licensure.
Minimum Qualifications
Knowledgeable in assessment, modalities, treatment planning and counseling techniques and working with comorbid populations (chronic health conditions, mental health, and substance use). Experience in the use of motivational interviewing, cognitive behavioral therapy, trauma informed care and strengths-based interventions. Knowledge of and confidence in assessing and diagnosing mental health conditions. Ability to interact and render service impartially to diverse populations. Interacts with respect and in a professional manner with patients, staff, external customers and maintains professional boundaries. Must be able to commute independently between CRCHC sites.
- Experience: 2 years professional experience working as an LCSW or clinical psychologist in an integrated medical clinic, or related health care setting
- Additional skills required: Knowledge of federal/state regulations and ethical standards for the delivery of Licensed Clinical Social Worker care. Excellent communication, coaching skills, and team oriented. Strong computer skills with knowledge of Microsoft Office products. Excellent verbal and written communication skills. Able to work independently and in a multidisciplinary team. Able to effectively utilize an electronic health record to document all patient encounters.

Education: Graduate of an accredited Master of Social Work (MSW) program.
Certification(s)/Licensure: Current and unrestricted state of North Carolina license as LCSW, required

Key Responsibilities
1. Provide diagnostic and brief therapeutic client-centered interventions, consultations, education and referral/linkages for individuals with emotional, behavioral, medical, mental health and substance abuse issues.
2. Collaborate with the interdisciplinary team in coordination of care and implementation of the treatment plan.
3. Counsel, educate, support and assist clients and families to gain access to internal and external resources.
4. Develop and maintain a working knowledge of local and regional external behavioral/mental health, substance abuse and psychiatric resources for clients in need of additional resources.
5. Participate in SBIRT program.
6. Meet or exceed established productivity expectations.
7. Awareness of personal biases and an ability to deliver culturally and trauma informed care.
8. Provide crisis interventions services.
9. Complete and submit documentation within 72 hours of an encounter.
10. Participate in peer supervision to ensure utilization of evidence-based treatment plans and maintain necessary workflow.
11. Provide education and in-service training to staff on topics related to communication and behavioral/mental health topics relevant to the populations served.
12. Participate in multidisciplinary team meetings, QI meetings and other community-based meetings, as assigned.
13. Partner with bilingual staff and interpreters to serve Spanish-speaking patients.
14. Participate in quality improvement activities by initiating or contributing to monitoring, measuring, analyzing, improving and/or controlling program goals, objectives and/or services.
15. Support current incentive, regulatory, and certification requirements (such as Grant funded initiatives, Meaningful Use, PCMH, and UDS) through documentation, participation in initiatives, and other activities as directed.
16. Perform other duties as assigned.
