What are the responsibilities and job description for the Case Manager - Clinical Resource Management position at Cabell Huntington Hospital?
The Case Manager exercises independent judgment and professional discretion in the practice of case management to a select group of patients. The Case Manager collaborates with attending physicians and residents and serves as advocate to help patients and families navigate the acute episode of care. The Case Manager monitors patient's continuing stay for resource utilization and helps prepare them for a safe and timely discharge. The Case Manager is proactive and uses collaborative problem solving skills to meet the needs of the patients and physician.
Requirements:- Bachelor's Degree in Nursing is required
- Master's Degree is preferred
- RN Licensure is required
- Minimum of 5 Years of Position-Related Experience- Minimum experience includes: patient care in the hospital setting, direct knowledge of care management, discharge planning and resource management.
