Cabell Huntington Hospital is seeking a full-time Nurse Practitioner to work in our Inpatient Palliative Care Department.
The Palliative Care Nurse Practitioner (NP) is responsible for planning, managing and evaluating patients with advanced chronic or life-threatening illness and making recommendations to the plan of care. The NP will respond to palliative care consultations and requests to improve symptom management, improve quality of life, clarify goals of care, and assist patients/families in understanding disease process, prognosis and options for care. The NP will work with the medical team, nursing, case management, and other members of the health care team to provide optimal outcomes. The NP will participate in palliative care multidisciplinary meetings to ensure continuity of palliative care across the continuum of care.
Palliative Care Nurse Practitioner will coordinate, implement and evaluate palliative care education needs for patients and staff which include functioning in the role as consultant and patient educator.
The shift will be determined by each hiring manager. The pay rate will be determined in accordance with the labor agreement.
