Cabinet Peaks Medical Center is looking for a Licensed Clinical Social Worker (LCSW) to join our team!
The LCSW will work collaboratively with an interdisciplinary treatment team which may include a family physician, nurse practitioner, PA, nurse, care coordinator, medical assistant, and a psychiatrist. This role will provide consultation and brief, short term, solution-focused assessment and interventions to patients in order to improve overall function and health using a bio-psycho-social approach.
The LCSW will be caring for the community through scheduled and same day walk-in referrals and will involve problem solving with the primary care team. Previous experience working in mental health settings will be helpful, but training and acquisition of skills in this new and evolving model of behavioral health integration will be crucial.
Major Job Duties & Responsibilities
Support the mental and physical health care of patients on an assigned patient caseload. Closely coordinate care with the patient’s medical provider and, when appropriate, other mental health providers.
Screen and assess patients for common mental health and substance abuse disorders. Facilitate patient engagement and follow-up care (In person or by telephone).
Provide education to patients and staff about common mental health and substance abuse disorders and the available treatment options.
Systematically track treatment response and monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications.
Provide brief behavioral interventions and/or psychotherapy using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate.
Provide or facilitate in-clinic or outside referrals as clinically indicated.
Participate in regularly scheduled (usually weekly) caseload consultation with the psychiatric consultant and communicate resulting treatment recommendations to the patient’s medical provider.
Document patient progress and treatment recommendations in EHR and other required systems so as to be shared with medical providers, psychiatric consultant, and other treating providers.

Experience
Two to three years experience working with mental health patients preferred.
Experience providing individual and group therapy preferred (Especially brief therapies such as MI, CBT, ACT, PST, Behavioral Activation).
Educational Requirements
Master’s degree in Social Work.
Current LCSW Montana license.
Schedule
Monday-Friday with call requirements.
