Job Title/Position: Certified Home Health Aide CHHA
Reports To: Team Leader
Schedule: Wednesday - Sunday
JOB DESCRIPTION SUMMARY
The Certified Home Health Aide is an integral member of the interdisciplinary group who works under the supervision of a registered nurse and performs various services for a patient as necessary to meet the patient’s personal needs and to promote comfort. They are responsible for observing the patient, reporting these observations, and documenting observations and care performed.
Responsibilities:
- Performing personal care needs after given a patient's assignment and attending to his/her requests promptly.
- Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, Oxygen etc.)
- Assisting in patients ADL's
- Taking and recording temperatures, pulse, respirations, and blood pressure when ordered with appropriate skill and competency.
- Reporting on patient's condition and significant changes to the RN Case Manager.
- Providing respite for patient's family/caregiver when on-site, as appropriate.
- Adhering to the organization's documentation and care procedures and standards of personal and professional conduct.
Requirements:
- Current California Certified Home Health Aide license.
- Ability to communicate with staff in person and via phone, maintain patient charts.
- Intermediate proficiency in EMR system.
- Current CPR certification.
- CA licensed driver with an insured automobile.
- Ability to work with little direct supervision. Secure with issues of death/dying. Provides a calm manner when in a patient’s home.
- Demonstrates tact, patience, and good personal hygiene.
