Job Summary
We are seeking a dedicated and detail-oriented MDS Coordinator to join our healthcare team. The ideal candidate will play a critical role in ensuring accurate assessments and documentation for patients, particularly in skilled nursing facilities. This position requires a strong understanding of the Minimum Data Set (MDS) process, as well as experience in medical management and coding. The MDS Coordinator will collaborate with interdisciplinary teams to enhance patient care and ensure compliance with regulatory standards.
Duties
- Coordinates the process for completion of the MDS /RUGS information within the Nursing Department and in conjunction with other departments and services.
- Responsible for completion of the nursing component and for certification of completion of the RAI (Resident Assessment Instrument).
- Responsible for full house and quarterly PRI Instrument.
- DUTIES AND RESPONSIBILITIES
Supports the Mission Statement and philosophy of Saint Cabrini Nursing Home. Supports and promotes philosophy, objectives, policy and procedures of the nursing department.
- Obtains from Health Information Systems a list of all Residents who are scheduled for annual or quarterly reviews.
- Updates list with an addendum listing new information, especially new admissions and significant changes.
- Assists the professional nursing staff in completing the nursing component of the resident assessment..
- Verifies completion of the resident assessment, triggers and protocols and attests this by signature.
- Contributes to the interdisciplinary care plan by communicating any resident issues that are pertinent and by monitoring the nursing department’s responsibility of care plan goals for timely achievement. .
- Coordinates and evaluates each resident in conjunction with other Assessors for reimbursement purposes during the months indicated for CMI.
- Monitors documentation pertaining to MDS for compliance purposes.
- Attends seminars, workshops, etc. to keep abreast of current changes and trends in the health care field with emphasis on case management and managed care contracts.
- Assists with implementing recommendations of the Quality Improvement Program as they relate to Resident care planning.
- Prepares and submits reports as requested.
- Performs other related and unrelated tasks as required or indicated.

QUALIFICATIONS
EDUCATION
- Graduate of an approved school of Nursing
- Thorough knowledge of MDS and RUGS requirements as well as Federal and New York State requirements
- Commitment to maximizing reimbursement
- MDS Rugs Technical Training
- EXPERIENCE
- Minimum of 5 years experience in Long Term Care
- Knowledge of nursing theory and practice
- SPECIAL SKILLS
- Ability to communicate effectively both written and oral, with staff, visitor etc.
- Strong clinical skills
- Ability to use computer.
- Exceptional computation and analysis skills
- Skilled in Time Management.
- Microsoft Office Proficiency
- Flexibility in expanding role as indicated
- LICENSES/REGISTRATIONS/CERTIFICATIONS
Current NYS license to practice profession of registered nurse.
- Current NYS PRI Assessor number
Job Type: Full-time
Pay: $54.00 - $57.00 per hour
Expected hours: 37.5 per week
