Cogir Senior Living (formerly Cadence) is hiring a compassionate and dedicated LPN/LVN Wellness Nurse for our premier retirement community, Cadence Millbrae.
Our Wellness Nurse (LPN/LVN) plays a vital role in ensuring the health and well-being of our residents. This role provides high-quality care through health assessments, medication management, and coordination with residents, families, and the care team. Responsibilities include initial and subsequent resident health assessments, monitoring resident health, addressing changes in condition, staff training and supervision, and promoting a safe and supportive environment that aligns with our mission to provide exceptional care.
The position is full-time; the schedule is Tuesday through Saturday.

What will you do as a Wellness Nurse - LPN/LVN?
- Perform resident assessments and reassessments according to policies, monitoring any changes in condition.
- Communicate with the Health and Wellness Director about resident needs to ensure consistent quality services.
- When necessary, participate in hands-on care, medication administration, and specific nursing tasks.
- Schedule and assign caregiving and med tech shifts to ensure adequate coverage and compliance with staffing guidelines and department allowances.
- Supervise, train, and support care team members, promoting teamwork and adherence to care standards.
- Assist in the transcription of physicians' orders and the review of new physicians' orders.
- Maintain accurate and up-to-date resident records, including care plans, incident reports, and medical documentation.
- Coordinate care planning with home health agencies on-site, working within community and company policies.
- Address residents' special needs, requests, and complaints.
- Coordinate admissions with the sales team, including supervising move-ins.
- Understand the community's care regulations along with state-specific regulations.
- Facilitate communication between residents, families, and healthcare providers to ensure seamless care delivery.
If you have these qualifications, we'd love to chat:

- Valid, unencumbered nursing license (LPN/LVN).
- At least three (3) years of experience in geriatric nursing, including experience with medication administration, delivery systems, and pharmacies, preferably in Assisted living and memory care.
- At least one (1) year of relevant supervisory experience, including staff scheduling and management, is preferred.
- Knowledge of dementia and Alzheimer's is preferred.
- Excellent attention to detail and high integrity.
- Excellent communication skills and teamwork mindset.
- Strong leadership and team management skills.
- Outstanding organizational skills.
- Positive attitude, empathy, patience, and commitment to treating our residents with dignity and respect.
- Ability and willingness to participate in on-call rotation.
- Must meet all mandatory health requirements according to state regulations.
