LOOKING FOR STARS TO HIRE A SUPPORT COORDINATOR TO WORK WITH ELDERLY POPULATION PARTICIPANTS IN ONE OF OUR VARIOUS SUPPORT COORDINATION AGENCY ROLES.
SALARY IS NEGOTIABLE FOR INDIVIDUALS WITH EXPERIENCE AS A SUPPORT COORDINATIOR AND WHO IS MDS/IHC CERTIFIED.
$3,500 SIGN ON BONUS FOR APPLICANTS WHO IS MDS/IHC CERTIFIED AND HAVE PRIOR SUPPORT COORDINATION EXPERIENCE. BONUS PAID AFTER SATISFACTORY COMPLETION OF THE INTRODUCTORY PERIOD.
A Support Coordinator is a person with a passion for helping people and a desire to make a difference in the lives of individuals who live in their community. We serve to coordinate and connect individuals with various physical or developmental disabilities with services that enable them to live safe, inclusive, and fulfilling lives in their own homes as opposed to living in a nursing home or other restrictive setting. We advocate for each individual and monitor the services the elect to have provided.
Applicants must be excellent communicators. You must be proficient in using the computer and have excellent writing skills. Support Coordinator Supervisors review very person centered plans of care and other documents for the people we serve. Excellent organization, critical thinking, problem solving, and interviewing skills are necessary to be successful in this position. Applicants must work well under pressure and be able to meet the many deadlines associated with case management.
The perfect candidate MUST possess a Bachelor Degree in a human service field such as Child and Family Studies, Sociology, Criminal Justice, Psychology, Nursing, etc. A degree in General Studies or Education is accepted with 16 credit hours in human service coursework. Must have transportation, a valid driver’s license and maintain current motor vehicle insurance at all times.
