What you'll do :
As the Social Services Assistant , you are responsible for :
As the Social Service Assistant, you will work with residents and families to assist in providing services and ensure they have access to the services to meet their needs.
Duties :
Assisting in the development and implementation of client care plans.
Maintaining accurate and up-to-date client records and case files.
Conducting preliminary client interviews and assessments.
Coordinating with other service providers and agencies to ensure comprehensive care.
Providing practical and emotional support to individuals and families.
Qualifications
At least 1 year of Long-Term Care experience is preferred.
At least 1 years of experience in customer service.
Excellent judgment, analytical, and interpersonal skills, including written and verbal communication skills to effectively communicate with all departments.
Excellent note taking skills.
Knowledge of Medical Terminology required.
Ability to work independently and within cross-functional teams.
Excellent calendar management skills, including the coordination of complex executive meetings.
Ability to research, analyze data and solve problems effectively.
