Job Description: We are seeking a highly skilled Healthcare Professional to join our team in Delaware. As a key member of our healthcare staff, you will be responsible for providing community-based care coordination and management services to our clients. This is a full-time hybrid role that requires flexibility and a strong commitment to delivering high-quality patient care.
Key Responsibilities:
- Conducting face-to-face needs assessments and telephonic support to members and their caregivers.
- Developing and implementing individualized plans of care that address the member's specific needs and goals.
- Coordinating care across the continuum of services, including physical, behavioral, long-term services and supports (LTSS), social, and psychosocial needs.
- Facilitating referrals to special programs, such as Disease/Chronic Condition Management, Behavioral Health, and Complex Case Management.
Requirements:
- Current Registered Nurse licensure in the state of Delaware.
- 2 years of experience in case management and discharge planning, preferably in a facility setting.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Ability to travel throughout the state, with occasional overnight stays.
