Reports To: Administrator/Director of Health Care Services
Exempt Status: Non-Exempt/ Full Time / Part Time
General Description of the position:
The Caregiver is responsible for helping an assigned number of residents with their activities of daily
living and assisting the Director of Resident Care Services with supplies, preparation and appropriate
charting.
Essential functions:
Assist in training of new staff as supervisor assigns.
Assist with bathing, grooming and hygiene, dressing, personal laundry, toileting, escorts and reminders for those residents who require those services and as assigned on the task sheet.
Note and check on residents who miss meals.
Attend all mandatory trainings to earn no less than 16 hours per year.
Follow HIPPA guidelines.
Learn proper use of fire extinguisher and know what your responsibilities are in case of fire.
Keep a positive up-beat attitude and maintain open communication between supervisor and other personnel.
Help maintain a safe, pleasant and clean environment.
Monitor, address and share in a timely manner with the Administrator/Director of Health Care Services all resident’s and family needs and/or concerns.
Notify Administrator/Director of Health Care Services of changes in resident’s health, dietary needs and behavior.
Read 24-Hour Communication log for all time lapsed since last shift and enter daily shift-to-shift report notes with off-going and on-coming shifts.
Responsible for daily charting and follow-up procedures. Daily charting in service schedule and Communication Log.
Write incident reports and follow-up with report to Director of Health Care Services before shift ends for incident reports.
Keep close monitor with “At Risk” residents and complete appropriate follow-up charting.
Be trained on Service plans.
Emergency Care and Procedures—Assess emergency and handle paperwork to send to keep in charts.
Follow-up on any complaints made from resident or anyone else regarding resident’s Bill of Rights and report to Director of Health Care Services.
Keep vitals up-to-date and taken as often as ordered or necessary.
File paperwork in the appropriate place and in a timely manner.
Welcome new residents. Get to know their likes and dislikes, and what is important to the family and residents in terms of care needs.
Encourage residents to be independent.
Perform other duties as assigned by the Director of Health Care Services.

Routine Tasks:
Peri-care on females (vaginal secretions).
Handling soiled linen.
Clean up of potentially contaminated fluids.
Attending to an injury where blood is present.
I understand that because of the above-mentioned routine tasks, I could be at potential risk for contracting a bloodborne pathogen, including but not limited to: non-A hepatitis, non-B hepatitis, hepatitis B (HBV), delta hepatitis, syphilis, malaria and/or human immunodeficiency virus (HIV).
Therefore, the company will provide me with the appropriate training in Universal Precautions and OSHA’s Exposure Control Standards to minimize any possible exposure. They will further provide me with an HBV vaccination if I so choose.
It will be my responsibility to adhere to all Exposure Control Policies as set forth by the company
I have read the above and have received training on Bloodborne Pathogens.
Working conditions:
Sitting/Standing: up to 8 hours
Lifting requirements: up to 200 lbs. with assistance.
Knowledge, skills and abilities required:
Must have excellent communication skills both written and verbal, with Residents, their families and other staff members and supervisor.
Equipment used:
Office equipment including but not limited to: telephones, computers, copiers and printers.
Occasional tasks may require the use of (but not limited to) lifts.
Qualifications:
A genuine concern for the elderly.
Must perform excellent customer service skills

Capable of making mature judgments.
Remain drug free.

Ability to pass CPR.
Must have approved 65-hour pre service training certificate.
Must have First Aid Training Certificate.
Must have abdominal thrust training certificate
Ability to maintain confidential information.
Dependable.
Education:
High School graduate preferred.
