Supportive Housing Project - Strengths-Based Case Manager

Summary:
We are seeking a dedicated Strengths-Based Case Manager to join our team. As a Strengths-Based Case Manager, you will play a crucial role in supporting individuals to achieve their goals by identifying and leveraging their strengths. This role requires compassion, organizational skills, and a deep commitment to empowering residents through a strengths-based approach.
Key Responsibilities:
- Conduct comprehensive strengths assessments for residents to identify their capabilities, interests, and resources.
- Develop individualized strengths-based service plans in collaboration with residents and Louisville Metro Housing Authority (LMHA).
- Coordinate and facilitate client access to necessary services, resources, and supports based on their strengths and needs.
- Provide ongoing support and advocacy for residents to ensure they are connected with appropriate services and making progress towards their goals.
- Maintain accurate and up-to-date documentation of client interactions, progress notes, and service plans in accordance with organizational policies and standards.
- Collaborate with the Clinical Case and Brokerage Case Managers, community agencies, and LMHA to advocate for residents and ensure comprehensive care.
- Monitor and evaluate client progress regularly, adjusting service plans as necessary to promote positive outcomes.
- Provide crisis intervention and support as needed, adhering to established protocols and procedures.
Qualifications:
- Bachelor’s degree in Social Work, Psychology, Counseling, or related field (Master’s degree preferred).
- Proven experience in case management or social services, particularly utilizing strengths- based approaches.
- Strong understanding of trauma-informed care, cultural competence, and ethical guidelines in social work practice.
- Excellent communication skills, both verbal and written, with the ability to engage effectively with diverse populations.
- Ability to work independently and as part of a multidisciplinary team, demonstrating flexibility and strong problem-solving skills.
- Proficiency in relevant computer applications and software, with the ability to learn new systems as needed.
Preferred Skills:
- Experience working with specific populations (e.g., children, elderly, individuals with disabilities).
