- DMV Printout (DMV driving record dated within 30 days of application)
- Copy of most recent Department of Education T0-1 Training Certificate
- First Aid Certification (First Aid Certificate, if the first aid test was waived by CHP [must be American Red Cross or other EMS authority approved])
- Proof of HS Graduation (Proof of HS Graduation (acceptable formats : high school diploma or equivalent; college degree; transcripts [must show name and graduation date].
