Position Overview
We seek a committed and culturally responsive Health-Related Social Needs (HRSN) Screener to conduct screenings with Medicaid Members in Erie and Niagara Counties. This part-time position plays a vital role in identifying and addressing social determinants of health, including housing, food security, transportation, employment, education, and interpersonal safety. Through virtual and in-person engagement, the Screener will assess Members’ needs, connect them to resources, and help eliminate barriers to care and stability.

What You’ll Do
Screening & Engagement
- Conduct 1:1 screenings (in-person, phone, or virtual) using the Accountable Health Communities (AHC) HRSN Screening Tool
- Ensure timely screenings for all eligible Medicaid Members, including re-screenings following major life events
- Engage participants respectfully and confidentially, honoring cultural backgrounds and lived experiences
Documentation & Referral
- Accurately enter screening data into the SCN IT platform or other approved systems
- Identify Member needs and make referrals to appropriate services within the Social Care Network
- Monitor and track follow-ups and re-screenings
Collaboration & Support
- Collaborate with Navigators, case managers, and community partners to ensure seamless support for Members
- Provide Members with information on available community resources and assist with access
- Participate in training, team meetings, and quality assurance efforts
What We’re Looking For
Educational Requirements & Experience
- High school diploma or GED required
- Associate’s or Bachelor’s degree in Social Work, Public Health, Human Services, or a related field preferred
- At least 1 year of experience in healthcare, social services, case management, or community outreach
- Familiarity with Medicaid and social determinants of health is a plus

- Proficiency with electronic data entry systems
- Bilingual (English/Spanish or other languages) candidates are strongly encouraged to apply
Work Environment & Additional Information
This hybrid role includes remote work and in-person travel across Erie and Niagara Counties. Screenings may occur in client homes, community centers, hospitals, clinics, or virtually. Reliable transportation is required. Flexible scheduling is available based on client needs and team coordination. Two part-time positions are available. Training will be provided for this position.
