What are the responsibilities and job description for the Hospice RN Case Manager position at Calstro Hospice Inc.?
Coverage area includes Hemet, Menifee, Temecula, Perris, Moreno Valley, San Jacinto, Beaumont, Banning, and surrounding cities.
Schedule: Monday - Friday 8:30am - 5pm
The RN coordinates hospice care with other team members and patients/families to ensure that the patients’ needs are being met. The Case Manager consults with the attending physician and Hospice Medical Director concerning changes in patients’ conditions requiring order changes and/or changes in the plan of care.
Responsibilities:
- Completes an initial, comprehensive and ongoing assessments of patient and family
- Oversees Plan of Care for accuracy and appropriateness
- Prepares discharge summaries
- Coordinates and conducts IDG meetings
- Oversee clinical supervision of nursing personnel; LVN’s and Home Health Aides
- Supervise the maintenance of medical records assuring accuracy, completeness and compliance
- Explains nursing and other Agency services to patients and families as a part of planning of care
- Adheres to Federal, state and accreditation requirements including Medicare and Medicaid regulations
- Completes own nursing visit notes as well as audit other nursing staff notes
Required Qualifications:
- At least 1 year experience in Hospice within that last 3 years
- Current California RN license
- Excellent written and verbal communication skills
- Valid CA Driver’s License and auto insurance
- Current CPR
- Current TB/Physical
- Pass Background Clearance
Preferred Qualifications:
- Bilingual (English/Spanish)
- Baccalaureate degree in nursing
PHYSICAL REQUIREMENTS
1. Prolonged periods of sitting at a desk and working on a computer or tablet.
2. Prolonged periods of standing and moving may be required.
3. Must be able to lift objects in excess of 20 pounds.

Pay: $51.00 - $56.00 per hour
