This is an in-office, M-F 8:30am - 5pm position in Montclair, CA. You must have an active LVN license.
Patient Care Coordinator
As a Patient Care Coordinator, you will play a vital role in ensuring the efficient and effective delivery of hospice services. Join our team and help us provide exceptional, patient-focused care. In this role, you will be responsible for:
- Patient Support: Answering phone calls and assisting hospice patients and their families with scheduling appointments, medication refills, and more.
- Scheduling Management: Managing and maintaining the calendars for LVNs, RNs, CHHAs, and on-call staff.
- Care Coordination: Collaborating with medical staff to schedule PRN visits as needed.
- Data Management: Inputting and updating patient information in the EMR system.
- Customer Service: Providing excellent customer service, addressing patient inquiries, and resolving concerns.
- Department Collaboration: Working with other departments to coordinate patient care and scheduling needs, including physical therapy, wound care, and other services.
- Inventory Management: Ordering and maintaining inventory of DME while implementing cost-saving measures.
- Administrative Tasks: Completing prior authorizations, verifying benefits, and preparing charts for billing to ensure timely reimbursement.

POSITION QUALIFICATIONS
1. Licensed Vocational nurse with current licensure to practice professional nursing in the state.
2. Previous hospice experience (1 year) preferred. At least one (1) year experience in patient care coordination required.
3. Bilingual preferred.
4. Proficient in computer skills, including MS Office and HospiceMD.
5. Demonstrates autonomy, organization, assertiveness, flexibility, reliability, and cooperation in performing job responsibilities.
PHYSICAL REQUIREMENTS
1. Prolonged periods of sitting at a desk and working on a computer or tablet.
2. Prolonged periods of standing and moving may be required.
3. Must be able to lift objects in excess of 20 pounds.
Job Type: Full-time
Pay: $30.00 - $36.50 per hour
