Summary Under the general supervision of the Health and Human Services Director, assesses, plans, implements, directs, evaluates, and supervises Calumet County Public Health programs and services consistent with Wisconsin State Statutes, Administrative Rules, CMS, and local policy.

Provides public health expertise and leadership to the Health Division.
Collaborates with the Health and Human Services Director to establish annual public health priorities and goals, and to identify key program performance indicators, measure them over time, and analyze them for suggested changes.
Provides leadership to drive the development of the Community Health Assessment, the Community Health Improvement Plan, and the public health strategic plan.
Leads the agency’s quality improvement, performance management, strategic planning, and workforce development teams.
Monitors the status of the community to identify health needs. Develops, implements, evaluates, and revises public health programs to mitigate or eliminate identified health needs.
Delivers targeted, culturally appropriate information to help individuals and groups understand health promotion and disease prevention information.
Ensures the delivery of public health nursing services is evidence-informed, best practice, and builds the scientific basis of public health.
Writes and updates internal public health policies and procedures; assists with policy development within the County government and community.
Monitors public health policy issues at the federal, state, and local level and coordinates with public health professionals from other agencies and jurisdictions, including, but not limited to, the Wisconsin Department of Public Health (DPH) and the Centers for Disease Control and Prevention (CDC).
Ensures program compliance with federal, state, and agency statutes, codes, standards, policies, and procedures.
Provides oversight of community health education and public relations. Builds relationships with community agencies and members who are committed to improving the health of the community. Serve as a member and possible facilitator of local community partnerships.
Represents Calumet County Public Health in meetings including, but not limited to: Health and Human Services Board meetings, regional health officer meetings, regional CAHL (Community Action for Healthy Living) committee meetings, and the FVHERC (Fox Valley Healthcare Emergency Readiness Coalition) meetings.
Directs and approves the Health Division’s planning, training, and exercising activities to prepare for public health emergencies. Serves as the Health Division representative to the Emergency Operations Center during Public Health Emergencies. Acts as a member of the Incident Command System.
Functions as vaccine coordinator. Assures proper inventory, transportation, and storage of vaccine. Assures all requirements of the Vaccine for Children (VFC) program are met. Leads preparation for state and federal site visits and audits. Participates in local immunization coalition meetings and initiatives. Serves as Wisconsin Immunization Registry and Wisconsin Electronic Disease Surveillance system administrator for new users.
Leads the public health accreditation readiness activities.
Leads public health staff in establishing annual goals and action plan. Organizes, distributes, directs, and delegates work to staff fairly and impartially. Provides clear and appropriate directions to employees. Provides training and coaching to staff. Ensures that staff have the resources and flexibility to learn and perform their jobs and improve their job performance.
Leads and facilitates public health staff meetings and work groups and ensures proper accountability of all program staff.
Coordinates and/or delegates in-service training and continuing education for public health staff; coordinates student experiences within the Health Division.
Supervises the purchase, arrangement and repair of all Public Health equipment and supplies.
Provides information to the Health and Human Services financial manager to develop the annual budget for Public Health programs.
Works with the Health and Human Services fiscal staff to prepare grant and programmatic budgets, and monitors and oversees grants and budget activity for public health.
Oversees grant writing for grant funding, including the consolidated contract process, and monitoring of secured grants (data collection, written reports) as needed for each program.
Assists with collecting information for the annual report, state reports, and grant reports.
Functions as the County Health Officer in accordance with Wisconsin §251.06.
Enforces state public health statutes and rules, any regulations that the local board of health adopts, and any ordinances that the Health and Human Services board enact if those regulations and ordinances are consistent with state public health statutes and rules.
Lead investigator for human health hazard investigations and orders for abatement.
Issues isolation and quarantine orders as needed for communicable disease containment.
Responsible for communicable disease follow-up in our jurisdiction.
Lead for communicable disease policy questions for our jurisdiction and for county employees.
Oversees and assures that Public Health meets appropriate professional standards, including the minimum required to be a Level II Wisconsin Health Department.
Reports to work as called in 24/7 in a public health crisis or emergency and performs public health emergency response duties as assigned and consistent with training provided.
May perform vaccination or TB skin testing PRN.

Supervisory Responsibilities
Directly supervises Public Health employees in the Health Division. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems; approves leave and timecards.

Minimum Qualifications (Education/Experience)
A bachelor's degree from a nursing program accredited by the national professional nursing education accrediting organization or from a nursing program accredited by the board of nursing, either of which shall include preparation in public health nursing; minimum of three years’ experience in a full-time position in a public health agency with supervisory experience preferred; or an equivalent combination of experience and training which provides the required knowledge, skills, and abilities.
Preferred Qualifications
Master’s Degree in Nursing from a program accredited by the National Professional Nursing Education Accrediting Organization, either of which should include public health nursing OR, a Master’s Degree in public health, environmental health, the physical or biological sciences, or a similar field with a minimum of three years supervisory experience in public health with Public Health courses, preferred.
Minimum of one year training and experience in public health emergency preparedness, including ICS (Incident Command Structure) and NIMS (National Incident Management System).
Management and leadership experience.
Knowledge of emergency preparedness and response.
Experience with quality improvement and performance management.
Certificates, Licenses, Registrations
Current licensure from the State of Wisconsin as a Registered Nurse.
Current Wisconsin Driver’s License, with access to a dependable automobile and carrying at least the minimum automobile insurance coverage.

The benefits listed below represent an employee who works full-time (100%). Regular part-time employees working half time or more per week may be eligible to receive employee benefits on a prorated basis. Qualifying employees working an average of 30 hours per week are eligible to receive health insurance coverage at the same premium contribution level as regular full-time employees. The percentage of employment which has been established by the budget or Board, shall determine the benefit pro-ration, with the exception of Home Health Aides, who will be pro-rated in July of each year based on the percentage of full-time hours worked in the previous twelve (12) months.
1. PTO. (Paid Time Off). Calumet County's PTO Plan combines vacation, sick leave, holiday, and bereavement leave into a single leave plan – PTO. Newly hired full-time employees will be credited with six (6) days of PTO immediately upon hire. The accrual will be based on the employee's length of service with the County and the hours paid in a pay period. PTO may be carried over from year to year provided the balance does not exceed the maximum allowable amount.
2. ELB. (Extended Leave Bank). The Extended Leave Bank is a bank of time separate from PTO that is intended for use in situations where the employee is absent from work due to medical necessity lasting more than three (3) days, or for events qualifying under the Family and Medical Leave Act (FMLA).
3. Holidays. The official County holidays are factored into the PTO Accrual as follows: New Year's Day, Friday before Easter, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Day After Thanksgiving, the Day Before Christmas Day, Christmas Day, and the Day Before New Year's Day.
4. Insurance. Employees eligible to participate in a High Deductible Health Insurance Plan. The County pays ninety (90%) percent of the single or family premium for full time employees who complete the Wellness Program criteria. To assist in meeting deductibles, the County will also contribute, to an HSA, for eligible employees. Dental and Vision Insurance is also available.
5. PEHP. (Post Employment Health Plan). The County provides a PEHP for regular full-time and eligible regular part-time non-represented employees. The PEHP is a multi-employer trust (IRS Code 501(c)(9)). The plan allows for a tax-free pre-funding of post employment medical expenses. The County contributes a biweekly flat dollar amount per employee. Contributions will be allocated to the universal reimbursement sub-account.
6. Life Insurance. The County provides a life insurance policy to qualifying employees equal to the estimated calendar year earnings of the position, with the County paying the premium. Additional units of insurance are also available along with spouse and dependent coverage, with the employee paying the full premium.
7. Disability Insurance. Long-term and short-term disability insurance is available, with the employee paying the full premium.
8. Retirement Plan. The County provides a pension plan through the Wisconsin Retirement System. The County pays 50% of the actuarially required WRS contributions for qualifying general employees.
9. Section 125 Flexible Spending Program. The County provides a Section 125 Flexible Benefits Program wherein the employee may set aside pre-tax dollars to use for expenses not covered by insurance or for dependent care expenses.
10. Deferred Compensation Program. The County provides a Section 457 Deferred Compensation Program wherein pre-tax dollars can be deposited in an investment of the employee's choice through providers used by the County.
