Calvary Women’s Services offers housing, health, education, and employment programs that empower homeless women in Washington, DC, to transform their lives.
JOB SUMMARY :
The Case Manager is a member of the program staff team with responsibility for offering case management services and personal support to women living in Calvary’s housing programs. The Case Manager works from a strengths-based, trauma-informed care model, offering resources to support women in achieving their goals for health, financial stability and independent living. The Case Manager works primarily on-site at the housing programs, but also attends off-site meetings and appointments with clients.
ESSENTIAL DUTIES AND RESPONSIBILITIES :
Case Management
- Manage a caseload of up to 20 residents in Calvary’s transitional housing program for domestic violence survivors
- Develop and support personalized case plans to support women’s efforts to reach their goals
- Link clients with services available on-site including client savings program, medications management, LEAP programs, mental health services, Step Up DC job placement programs, and addiction recovery services
- Provide referrals for community-based services including medical care, benefits applications, addiction treatment, and life skills development
- Actively participate and provide hands-on support to residents as they make connections with housing and community-based services including attending meetings and appointments, assisting with documentation
- Coordinate with the Director of Intake and Housing to ensure a smooth transition for new residents
- Maintain current confidential case files and documentation (Apricot and hard copy) on each resident available for monthly review by the Program Manager
- Complete and submit monthly reports and client summaries
Client Services
Facilitate community meetings and participate in regularly scheduled social and educational activitiesCoordinate referrals and communications with therapists, nurse practitioner, RPG facilitator, Workforce Development Manager and Senior Manager for Education and ProgramsActively build and maintain relations with other service providers in the areas of mental health, housing, substance abuse and addiction recovery services; visit programs and take a hands-on approach when making resident referrals to other service providersWork with RPG facilitator / CAC on the completion of toxin screens for clientsActively gather and share information on related services and resources within Calvary’s program staff teamOTHER DUTIES AND RESPONSIBILITIES :
Meet with the Program Supervision for weekly supervisionParticipate in clinical supervision with the Clinical SupervisorComplete all contract mandated professional trainingParticipate in program staff meetings, all-staff meetings and in-service trainingWork on-call coverage and evening shifts as scheduledOther duties as assignedRequirements :
REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES :
Experience in case management, particularly working with women and the issues of substance abuse, mental health issues and homelessnessAbility to work as a team memberExcellent active listening and crisis intervention skills with ability to exercise and model good judgmentStrong interpersonal, written and oral communication skills; proven ability to communicate with diverse audiencesAbility to manage multiple tasks and priorities, including proficiency in balancing direct service responsibilities with administrative and data management responsibilitiesStrong administrative skills and proficiency in Microsoft Office Suite (Word, Excel, Outlook) requiredEDUCATION AND EXPERIENCE :
Bachelor's or Master's degree in social work / related field or relevant experience preferred; strong knowledge of the complexity of poverty and women's homelessness, mental illness, trauma, substance abuse, and health issues preferred.WORK ENVIRONMENT REQUIREMENTS :
Own transportation is required for trips out of the officeMove, lift, carry, push, pull, and place objects weighing less than or equal to 30 pounds without assistance.Reach overhead and below the knees, including bending, twisting, pulling, and stooping.Climbing : Ascending or descending multiple flights of stairs, using feet and legs and / or hands and armsStand, sit, or walk for an extended period
