Job Summary:

The Community Health Worker (CHW) will support value-based care (VBC) initiatives by enhancing care coordination, addressing social determinants of health (SDOH), closing care gaps, and improving the patient follow-up processes. The CHW will play a critical role in improving patient outcomes, maximizing reimbursement opportunities, and reducing health disparities across Calvert County.

Maryland Community Health Worker Certificate

1 years of relevant experience.

Other Requirements:

Maintains unit-specific and hospital competencies, mandatory learning, and any clinical certifications required in accordance with the Staff Education and Training policy GA-057 and/or any other department requirements.

Serve as a liaison between patients, healthcare providers, and community resources to enhance patient access to care.

Accurately document patient interactions, referrals, and follow-ups in MEDITECH.

Collaborate with care teams, practice managers, and IT staff to streamline processes.
