We are seeking an full-time and part-time experienced Licensed Vocational Nurse to join our home health and hospice team! You will be responsible for the assessment and treatment of assigned patients, based on agency policies and procedures, through the competent application of the nursing process. Responsibilities:
- Monitor changes in patient symptoms or behavior
- Communicate with collaborating care team and attend Interdisciplinary Team Meetings
- Educate patients and their families about the dying process
- Provide advice and emotional support to patients and their family members
- Performs client assessments based on agency standards of practice to ensure effective and appropriate home care services.
- Submits accurate documentation within 24 hours of visit.
- Participates in on-call coverage as needed
Qualifications:
- Previous experience in nursing
- Familiarity with EMR systems
- Ability to build rapport with patients
- Strong problem solving and critical thinking skills
- Ability to thrive in a fast-paced environment
- Active, unencumbered LVN license in Oklahoma required
- Current CPR certification preferred
EXPERIENCE REQUIREMENTS
- Minimum six months nursing experience
- Hospice experience preferred

Pay: $45,523.00 - $86,255.00 per year
