Job Summary:

Collaborates with patients, families, advocates, healthcare providers, and community resource representatives to promote
health and communicates available community support resources to at-risk populations presenting to the healthcare
system. Makes patient assignments to SW team for L5 and L4 and fulfills programming assignments

Education:
-
Position requires a Master's Degree in Social Work from an accredited college or university.

Registration/Certification/Licensure:
-
Position requires licensure in the state of Maryland - LMSW, LCSW-C, LCSW, LGPC or LCPC.

Experience:
-
Position prefers 3 -5 years related medical experience in an acute care setting with one year of clinical supervision, and
prefers relevant experience in Behavioral Health.

Other Requirements:
-
Maintains unit-specific and hospital competencies, mandatory learning, and any clinical certifications required in accordance with the Staff Education and Training policy GA-057 and/or any other department requirements.

Effectively communicates with all patient populations and staff by implementing organization-wide communication
techniques as a daily practice. -
Effectively advocates for quality patient safety and satisfaction as a daily practice.

Demonstrated proficiency of computer skills necessary to effectively complete position requirements.

Interviewing and screening SW candidates

Previous experience and demonstrated professionalism in effectively communicating with customers and in promoting a
positive service atmosphere. -
Ability to work independently in a multi-task environment producing quality work.
