Job Summary
We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will be responsible for coordinating patient care, advocating for patients, and collaborating with healthcare providers to ensure the best possible outcomes.
Responsibilities
- Develop and implement individualized care plans for patients
- Coordinate and monitor patient care from admission to discharge
- Conduct assessments to identify patient needs and develop appropriate interventions
- Collaborate with healthcare providers to ensure continuity of care
- Provide education to patients and families on treatment plans and self-care techniques
- Maintain accurate and up-to-date patient records
- Adhere to HIPAA regulations and maintain patient confidentiality
Qualifications
- Current RN license
- Minimum of 2 years of care management experience in areas such as Home Health
- Proficient in medical terminology and medical-surgical procedures
- Strong understanding of HIPAA regulations
- Excellent communication and interpersonal skills
- Ability to provide compassionate and comprehensive patient care
- Experience working in a clinic setting is a plus
Job Types: Full-time, Contract
Pay: $31.65 - $40.81 per hour
Expected hours: No less than 40 per week
