Targeted Case Manager - Foster Care & Pediatrics
Job Summary:
We are seeking a dedicated and compassionate Targeted Case Manager with experience in foster care and pediatric populations to join our team. The ideal candidate will provide comprehensive case management services to children and families involved in the foster care system, ensuring they receive appropriate medical, behavioral, and social support services.
Key Responsibilities:
Conduct comprehensive assessments of children and families to identify needs, strengths, and support systems.
Develop and implement individualized care plans, coordinating services to address medical, behavioral, educational, and social needs.
Provide ongoing case management, including monitoring progress, adjusting plans as needed, and advocating for necessary services.
Work collaboratively with foster families, biological families, social workers, healthcare providers, schools, and community organizations to ensure the well-being of the child.
Facilitate access to mental health, medical care, educational support, and social services to enhance stability and development.
Conduct home visits, school visits, and community outreach to provide support and ensure a safe and stable environment.
Document all case activities in compliance with state, federal, and agency guidelines.
Attend and participate in court hearings, case staffings, and interdisciplinary meetings as required.
Provide crisis intervention and emotional support for children and families navigating the foster care system.
Qualifications:
Bachelor degree in Social Work, Psychology, Nursing, or a related field
2 years of experience in case management, foster care, or pediatric services.
Knowledge of child welfare laws, trauma-informed care, and social services resources.
Experience with Medicaid, community health programs, and service coordination.
Strong advocacy, communication, and problem-solving skills.
Ability to work independently and as part of an interdisciplinary team.
Preferred Qualifications:
Experience with electronic health records (EHR) and case management software.
Familiarity with state foster care regulations and best practices.
