The Case Manager will work closely with tenants to develop and implement independent living plans, identify and connect them with community resources, and advocate for their needs.
Key Responsibilities
• Conduct initial intake and assessment of tenants' needs
• Create and maintain tenant files
• Develop and implement individualized service plans
• Connect tenants with community resources and services
• Advocate for tenants' needs and rights
Requirements
• Bachelor's degree (e.g., B.A., B.S.W.) and experience working with SPMI populations
• Ability to work independently and as part of a team
• Excellent communication and interpersonal skills
