We are seeking a skilled Case Manager to join our team at Apex Place, a series of residential buildings in Forest Hills, Queens. The successful candidate will work closely with tenants to develop and implement independent living plans, identify and connect them with community resources, and advocate for their needs.
Key Responsibilities
• Conduct initial intake and assessment of tenants' needs
• Create and maintain tenant files
• Develop and implement individualized service plans
• Connect tenants with community resources and services
• Advocate for tenants' needs and rights
Requirements
• Bachelor's degree (e.g., B.A., B.S.W.) and experience working with SPMI populations
• Ability to work independently and as part of a team
• Excellent communication and interpersonal skills
