We are seeking a highly motivated and experienced Case Manager to join our team at Apex Place, a series of three residential buildings located in the Forest Hills neighborhood of Queens. The successful candidate will be responsible for managing a caseload of clients, conducting intakes and assessments, and developing individualized plans to meet their needs.
