Job Description
Providence Centralia Hospital is seeking an experienced Case Manager RN to join our Care Management Unit for a 13-week contract. The position involves working 8-hour day shifts (07:00 - 16:30), 5 days a week.
Key Responsibilities:
- Perform case management duties, including utilization review and discharge planning
- Coordinate patient care across multidisciplinary teams
- Assess patient needs and develop individualized care plans
- Advocate for patient health and wellness while ensuring efficient resource utilization
- Work closely with hospital leadership, physicians, and nursing staff to ensure quality care
- May be required to float between various like units based on skill set
Qualifications & Requirements:
- Active RN License in the state of Washington
- Minimum 1-2 years of Case Management experience
- BLS Certification (AHA)
- Strong communication and organizational skills
- Ability to work collaboratively in a fast-paced hospital environment
Additional Information
- Floating Requirement: Travelers may be required to float to other similar units based on facility needs
- COVID-19 Vaccine: Not Required
- Local Travelers: Candidates with a permanent residence within 50 miles of the facility are not eligible for a travel contract
