- 8 : 00 AM – 5 : 00 PM or 8 : 30 AM – 5 : 30 PM (Full-time)

Position Summary :
We are seeking a skilled and dedicated Licensed Vocational Nurse (LVN) to join our team. In this role, you will provide high-quality care to elderly and disabled participants, ensuring their health and well-being while supporting our care team. You will be an integral part of a dynamic, multidisciplinary team that values collaboration, patient advocacy, and education.
Key Responsibilities :
Participant Care :
Monitor participants' overall health, including vital signs and nutritional status.Administer prescribed treatments, medications, and vaccines per physician orders.Assist with personal care tasks, meals, and other daily living activities.Handle medical emergencies and provide ongoing wound care.Collect and document health data, ensuring timely assessments.Documentation :
Maintain accurate medical records, including medication administration and nursing care.File incident reports and communicate with families and agencies as needed.Coordination :
Assist in updating policies and procedures as required.Follow up on pharmacy reviews and communicate medication changes to healthcare providers.Train and evaluate CarePartners in collaboration with the Home Care Manager.Team Support :
Ensure safety standards are followed in the work environment.Mentor nursing aides, student interns, and volunteers.Attend staff meetings and trainings to promote team communication and development.Advocacy & Education :
Educate participants and families on health, safety, and medication management.Coordinate with physicians and external health providers to ensure participants' needs are met.Requirements :
Education & Experience :
High school diploma or GED.Completion of an accredited LVN program.At least 1 year of experience working with elderly or frail populations.Licensing & Certifications :
Current LVN license issued by the California Board of Vocational Nursing.CPR and First Aid certification (to be obtained within 6 months of hire).Valid California driver's license and personal vehicle insurance.Skills & Qualifications :
Strong communication skills, both written and verbal.Ability to work collaboratively within a team setting.Knowledge of healthcare regulations and the ability to apply them.Compassionate and patient-focused approach to care.Basic computer skills and familiarity with database management.Additional Requirements :
Successful completion of background checks (DOJ and FBI).Pre-employment physical and PPD skin test.Motor Vehicle Report verification.COVID-19 and Flu vaccinations preferred.Why Join Us :
