Shift: Evening Shift, 3x12 hours (36 hours guaranteed)
Job Description:
We are seeking a compassionate and skilled LPN (Licensed Practical Nurse) to join our long-term care (LTC) team for a 13-week contract position. The successful candidate will provide quality nursing care to residents in a skilled nursing facility, ensuring their health and well-being through effective care and communication.
Responsibilities:
- Provide direct patient care to residents, including administering medications, monitoring vital signs, and performing treatments as needed.
- Collaborate with the interdisciplinary healthcare team to develop and implement individualized care plans.
- Monitor patients' conditions and report changes to the RN or physician.
- Assist residents with daily activities such as bathing, dressing, and eating, promoting independence and dignity.
Requirements:
- Minimum of 1 year of experience as an LPN in a Long-Term Care (LTC) setting.
- Current and valid New York State LPN license.
- Basic Life Support (BLS) certification – required.
