Position Summary :

The Licensed Practical Nurse is responsible for providing direct nursing care and administering

medications and treatments within their scope of practice, under the direction of a Registered

Nurse. As a representative of the Nursing Department and of the organization, it is required that

the Licensed Practical Nurse perform the essential functions of the job in a professional manner,

with a positive attitude and is aware and supportive of the mission, vision, resident Bill of

Rights, policies, and procedures of the organization.

Resident Care Services

How Often

1. Provides direct nursing care to residents based on their individual needs. Daily

2.Provides care prescribed by the resident's primary physician and

consulting physician as indicated.

3. Maintains nutritional needs of the resident through assistance and

supervision.

4. Implements the resident's care plain in conjunction with the

interdisciplinary team.

5. Preforms other duties as assigned. As needed

6. Collects data on the resident that will assist in identifying the physical,

psychosocial, spiritual, and cultural needs of the resident.

7. Documents resident's care according to policy & maintains accurate

and accessible records and reports.

8. Communicates pertinent resident care information to Nurse Manager

and other resident care providers.

9. Maintains the resident's right to privacy, dignity, and individuality in all

care / activities performed.

10. Cares for resident clothing following facility policy and procedure,

making sure clothing is labeled appropriately.

11. Follows all State and Federal regulations regarding Abuse and

Resident's Bill of Rights including right to be free of pain, and HIPAA

guidelines.
