About the Role

The RN Assessment Specialist position requires a strong background in long-term care settings, with experience as an MDS Coordinator or Clinical Reimbursement Specialist. As a key member of our team, you will contribute to personalized resident care plans and ensure accurate data capture for services provided.

  • Valid State of CT RN License required
  • Advanced degree or certification preferred
  • Direct care experience in a long-term care setting, MDS Coordinator, Clinical Reimbursement Specialist or Nurse Assessment Coordinator experience
  • Knowledge of state and federal regulations governing MDS, Electronic Medical Record (EMR), PDP, MDS 3.0, Medicaid, and Medicare requirements helpful
  • Ability to work effectively in a multidisciplinary team environment and communicate clearly with various stakeholders
