The Regional Care Coordinator is responsible for providing support and guidance for care coordination functions within an assigned region of skilled nursing facilities, while maintaining direct care coordination responsibilities for designated facilities. This role manages a direct caseload at facilities as assigned by leadership, while ensuring that residents across all regional facilities receive comprehensive, high-quality care tailored to their individual needs. The Regional Care Coordinator will provide mentorship to the Care Coordinator team within the region, collaborate with healthcare providers, and develop strategies to improve patient outcomes and operational efficiency.
Essential Duties and Responsibilities
- Direct Care Coordination :
- Serve as the primary Care Coordinator for designated facilities within the region, typically those with lower skilled managed care volume
- Manage a direct caseload including comprehensive resident assessment, care planning, and discharge coordination
- Maintain accurate and timely documentation for assigned residents
- Coordinate directly with insurance providers for authorizations and continued stay reviews for assigned caseload
- Ensure compliance with all regulatory requirements and facility policies for assigned residents
- Leadership and Support :
- Provide guidance, mentorship, and professional development support to facility Care Coordinators within the assigned region
- Assist in developing and implementing policies and procedures to ensure effective care coordination practices
- Support and monitor care coordination practices to ensure compliance with regulatory standards and best practices
- Serve as a resource for complex care coordination situations and provide solutions-oriented guidance
- Patient Care Coordination :
- Manage direct caseload at assigned facilities while supporting facility Care Coordinators in the assessment, planning, implementation, and evaluation of patient care plans at other regional locations
- Ensure residents receive appropriate and timely care through both direct care coordination and oversight of facility care coordination processes
- Collaborate with interdisciplinary teams to facilitate comprehensive care and discharge planning
- Balance direct caseload responsibilities with regional support duties to maintain optimal resident care across all facilities
- Quality Improvement :
- Partner with facility Care Coordinators to identify areas for improvement in care coordination processes
- Implement strategies to enhance patient care and operational efficiency across the region
- Monitor patient outcomes and satisfaction, using data to drive quality improvement initiatives
- Ensure compliance with all federal, state, and local regulations related to care coordinator and patient care
- Monitor and improve payer authorization capture to support patient needs and reduce hospital readmissions
- Collaboration and Communication :
- Serve as a liaison between facility Care Coordinators, residents, families, healthcare providers, and external agencies
- Attend monthly meetings with health plans
- Support development of strong relationships with community resources to support residents' needs
- Facilitate effective communication between all stakeholders to ensure a seamless continuum of care
- Identify and secure funding opportunities to support care coordination programs and initiatives.
