Responsibilities:
- Deliver high-quality clinical services to patients in their homes, in accordance with federal and state regulations, and agency policies.
- Conduct thorough nursing assessments and develop individualized plans of care/service plans, in collaboration with other healthcare professionals.
- Evaluate patient needs and make necessary revisions to the plan of care, ensuring continuity of care from admission through discharge.
- Ensure patient eligibility and medical necessity for services, as defined by payer sources and agency policy.
Requirements:
- A minimum of one year experience as an RN is preferred.
- Current RN licensure in the state of practice.
- Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
