Position Overview:
As a Clinical Care Coordinator, you will play a vital role in ensuring the delivery of high-quality clinical care to our residents. You will assist the RN in overseeing clinical care assistants, providing training, mentoring, and continuous support to ensure the highest standards of care are met. Your leadership will help foster a collaborative and compassionate atmosphere, enhancing the overall well-being of our residents.
Job Responsibilities:
Ensuring that all Clinical Care staff have a comprehensive understanding of their roles, responsibilities
and checklist expectations.
Conducting thorough reviews of the new Clinical Care staff by verifying and documenting each
employees ability to demonstrate a complete understanding and practical competencies in their
duties outlined in the Clinical Care Preceptor Training checklist, prior to signing off on the employee
and uploading to HR.
Completing the required weekly checklist and ensuring all items are compliant and completed.
Completing the schedule and on-call calendar one month in advance, taking into consideration time-
off requests, shift trades, turnover, etc. and posting by the 15th of each month.
Updating Administration with any staff or resident care related concerns.
Complete all order processing via the electronic medical record system.
Review and following up on the Vitals Portal in the electronic medical record system.
Assisting the Clinical Team in completing medication ordering and cycle fill.
Compiling the podiatry list for upcoming podiatrist appointments.
Completing initial skin checks as deemed necessary by the ISP, physician orders, or Cambridge
Standing Orders, and reporting any concerns to the RN/LPN or designee.
Demonstrating infection control procedure by handling and disposing of linens, soiled clothing, or
supplies properly.
Communicate with families and MCOs on supplies needed for resident care.
Assist in obtaining monthly weights on all residents and documenting accordingly in the electronic
medical record.
Assist with providing education on charting compliance and appropriate nursing delegated tasks.
Assist with appropriate incident follow-up tasks.
Assisting residents in transportation to scheduled appointments, if necessary.
Providing support to the Clinical Care staff in checking inventory of medication carts and medication
room and regularly re-ordering anything not on cycle fill.
Providing support for the facilitation of communication between Clinical Care staff and Management
Team to ensure all communication is received/delivered by/to the Clinical Care Assistants and
Management Team.
Check Teams messages and Community Bulletin Board in PCC daily & ensure emails are answered
within 24 hours M-F.
Completing rounds at a minimum of daily to ensure CSL is maintaining a clean, homelike environment
that is free from hazards by tidying up spaces, cleaning up after residents, and ensuring that they
always look better than presentable.
Representing the community during any state or other required inspections, ensuring compliance, and
following-up with any deficiencies to ensure compliance is met immediately.
Responding promptly and working to resolve resident concerns and complaints.
Assisting residents with Activities of Daily Living (ADL's) to include but not limited to; bathing,
showering, dressing, grooming, shaving, nail care, hair care, eating, making bed, doing laundry,
ambulating, and helping to activities and other functions.
Completing proper documentation on weekly checklist.
Showing up for work on time and following the appropriate call-in and shift trading procedure, when
necessary, if applicable.
Responding to calls outside of regular work hours, including nights and weekends as necessary.
Motivating employees through recognition programs, training, and team building.
Maintaining a high standard and professional dress attire.
Completing the CBRF continuing education requirements per company and DHS guidelines.
Knowing & communicating the mission, vision, and values of CSL.
Greeting visitors and guests with a caring, compassionate manner.
Participating in mandatory staff in-services.
Assisting fellow team members in maintaining a clean, homelike environment that is free from hazards
by tidying up spaces, cleaning up after residents and ensuring that they always look presentable.
Demonstrating a passion for quality care as evidenced by interactions with co-workers, residents,
families, and visitors.
Performing other duties as assigned.
Follow rules and regulations outlined in DHS 83 and CSL Policies and Procedures that are not
specifically outlined above.
Qualifications:
Active Licensed Practical Nurse license in WI preferred but not required.
Active Medical Assistant Certification preferred but not required.
2 years experience in an Assisted Living community or similar setting/position.
CBRF Registry, CPR/AED certified, and Alzheimers/Dementia care experience preferred, but not
required.
