Clinical HIV linkage to care program coordinator provides administrative and clinical oversight and implementation of direct interventions and services to improve rates of linkage and retention to care in patients and their families served by the CAMC Ryan White Part C HIV Program. This position will provide specialized education and psychosocial services for Ryan White Program clients to include: clinical behavioral health assessment, diagnosis and treatment including individual and group psychotherapy and counseling to HIV-positive patients with high-risk behaviors, mental health problems and/or SUD. This position will also provide partner and contact testing and education, crisis intervention, implementation and coordination of services for referrals and other professional services as required.
Program coordinator provides field supervision for assigned staff, and provides direct clinical services to clients. This position also coordinates services with community partners to expand testing and outreach, DIS investigators, is a member of the CAMC MAT mobile services team, and ensures a continuum of care into medical and behavioral health services as well as supportive resources in the community. This position has a key role in assuring that quality measures are addressed and that the services under their management keep pace with the evolving needs of this population.
Responsibilities
1.Establish procedure and protocol for communication and coordination with all currently established HIV testing resources to achieve early linkage and retention in care; collaborate with community partners throughout service region to establish relationships and expand testing opportunities.
2.Ensure performance of rapid HIV testing and targeted counseling for partners/contacts of HIV positive patients, referral to PrEP as needed, and linkage to appropriate health services as needed.
3.Collaborate with patients to identify and address potential and/or existing barriers to engagement in medical care to include the biological, psychological, and social elements in their environments, and engage the client in treatment planning to increase retention to care. This includes clinical individual and group counseling.
4.Provide patient-specific health education and literacy training to ensure that all new patients are able to navigate the HIV system of care available to them, focusing on client-centered advocacy and self determination.
5.Ensure all newly confirmed positive patients have referral and linkage to internal and external resources (housing, meals, transportation, prescriptions, government entitlement program referrals, community resource referrals) to retain patients in care.
6.Assist patients and families with psychosocial adjustment to illness or injury, including: couple/family therapy, crisis intervention using established therapeutic processes, bereavement issues regarding loss, psychosocial support, support groups, establish treatment plans, etc.
7.Participate on RWP committees as needed, such as Quality Improvement, medical audits, patient care, etc.
8.Work as an integral part of the interdisciplinary team of assigned area.
9.Document all action done for or with the patient/family in Cerner and CAREWare.
10. Works in conjunction with the CAMC MAT mobile medicine team as services are expanded.
11. Provides on-call/crisis service coverage as required.
12. Adheres to NASW Code of Ethics.
