Perform and document psychosocial risk assessments, as delegated by a licensed Social Worker. Document conversations and interventions performed for discharge planning process, according to hospital policy and procedures. Assist with discharge planning. Provide courteous and prompt response to patient requests/inquiries. Gather and document basic objective data and report findings to the licensed Social Worker and/or Case Manager . Assist/participate in MDTR’s. Submit referrals within Ensocare, for DME/HH/SNF/Rehab, etc. Complete PAS assessment for nursing facility placement and enter into designated Utilization Management Contractor portal, within hospital and state guidelines

Responsibilities

1. Assist with referrals and coordinate discharge planning
2. Complete and document psychosocial risk assessments
3. Complete and submit PAS for nursing facility placement
4. Obtain choice forms
5. Discuss and assist with Advanced Directives
6. Participate in MDTR’s
7. Complete and submit referrals in Ensocare for DME, Home Health, SNF, Med Rehab, etc.
8. Arrange transportation assistance to facilitate discharge
9. Assist licensed Social Worker and/or Case Manager with discussion for discharge planning, including with patient/family/provider/care team
10. Arrange for community resources to facilitate discharge
11. Document all interventions/activity in the Medical Record
12. Assist with meds-to-beds or arranging for medication at discharge
13. Perform work in a cost-effective manner
14. Other duties as assigned

Knowledge, Skills & Abilities

Patient Group Knowledge (Only applies to positions with direct patient contact)

The employee must possess/obtain (by the end of the orientation period) and demonstrate the knowledge and skills necessary to provide developmentally appropriate assessment, treatment or care as defined by the department’s identified patient ages. Specifically the employee must be able to demonstrate competency in: 1) ability to obtain and interpret information in terms of patient needs; 2) knowledge of growth and development; and 3) understanding of the range of treatment needed by the patients.

Competency Statement

Must demonstrate competency through an initial orientation and ongoing competency validation to independently perform tasks and additional duties as specified in the job description and the unit/department specific competency checklist.

8. Follow organizational and departmental policies and procedures, as applicable.
9. Perform related duties as assigned.

Education

• Formalized Training (Required) Education: Enrolled in a Bachelor’s or Master’s level program for Social Work and classified as a senior or fourth year Experience: Must have completed at least 3 years of a bachelor’s level program and be classified as fourth year/senior level
