Camden Family Health currently has a full-time position for a LPN or CCMA care manager to work within an Office Based Medication Assisted Program.
Responsibilities:
- Provide care management services to patients in the Office Based Medication Assisted Program
- Coordinate with healthcare providers to ensure continuity of care
- Monitor patient progress and adjust care plans as needed
- Provide education and support to patients and their families
- Document patient interactions and maintain accurate records
- Collaborate with interdisciplinary team members to optimize patient outcomes
