YOUR ROLE & RESPONSIBILITIES
As a Social Worker, you will play a critical role in supporting the emotional and psychosocial well-being of our residents by:
Completing the Social Services portion of MDS assessments, care plans, and progress notes in compliance with all federal/state regulations and facility policies
Assisting residents and families in navigating social, emotional, and family-related concerns
Participating in interdisciplinary care planning based on residents’ needs and assessments
Maintaining timely and accurate documentation, including referrals to outside support agencies
Developing discharge plans and supporting smooth transitions for residents
Coordinating and hosting monthly Family Council Meetings
Engaging families and legal representatives in care planning and status updates
Collaborating with our mental health provider to ensure resident needs are met
Helping new residents adjust to long-term care through guidance and orientation
QUALIFICATIONS & REQUIREMENTS
To be considered for this role, candidates must meet the following qualifications:
Bachelor’s degree in Social Work or a related human services field (e.g., sociology, psychology, special education, or rehabilitation counseling)
Minimum of one (1) year of supervised social work experience in a healthcare setting
Prior experience in a long-term care or skilled nursing facility strongly preferred
Excellent verbal and written communication skills
Strong professionalism and the ability to maintain resident confidentiality
Ability to collaborate across departments and deliver outstanding customer service
