Description
This position is responsible for the coordination of the Family Services Planning Team (FSPT) ensuring that mental health and substance abuse treatment supports a consumer’s wellness goals. This position will also provide outreach to children and their families and provide psycho-education, problem identification, and linkage with community resources. This position will encourage, educate and engage prospective consumers who demonstrate an indication of substance abuse and mental health problems.
ESSENTIAL DUTIES AND RESPONSIBILITIES: to include the following: Other duties may be assigned.
- Responsible for being a community resource for children and their families.
- Coordinates linkage to services between providers and consumers.
- Ensures that treatment supports the consumer’s wellness goals.
- Organizes and tracks data in a timely and efficient manner.
- Schedules and coordinates FSPT staffings.
- Attends provider treatment team meetings as requested.
- Maintains contact with schools, service providers, state agencies, families and community partners.
- Identifies deficiencies in current service array and provides feedback to appropriate community agencies on identified gaps in services.
- Documents services, meetings and service plans in a timely manner.
- Understands and advocates the use of all state, local, and contracted funding streams.
- Confers with the FSPT Director on issues or concerns and documents responses in an appropriate and timely manner.
- Maintains a data base of community resources.
- Assists Case Managers with completing residential (SIPP) packets.
- Participates in outreach and training activities when requested and required.
Qualifications
Bachelor's degree preferred in a Human Service discipline from four-year accredited college or university and 1 year of experience working with child welfare children/adolescents and in compliance with State or contractual requirements.
Salary : $42,000
