Camelot Community Care a nationally recognized, multi-state provider of mental health, substance use, foster care and other social services to children, adolescents and teens, seeks a Case Manager for an innovative, Intensive Home-Based Treatment Program. Camelot Community Care is certified by the Ohio Department of Mental Health & Addiction Services (OhioMHAS) and accredited by the Council on Accreditation (COA).

Camelot Community Care seeks an Experienced Case Manager for an Intensive Home-Based Treatment Team that provides therapeutic services to individuals and families in their homes and community settings. As an IHBT Case Manager, you will have a shared caseload of 8-12 clients and work in collaboration with a qualified IHBT Therapist to provide treatment to clients with significant mental health concerns and family support needs. Your role will involve collaborating with the team therapist to best serve youth and families by: developing treatment plans, implementing therapeutic strategies and coordinating with the client’s treatment team to promote symptom management and stability within the family system.
Primary Duties:

Partner with clients and their families to develop individualized treatment plans based on assessment findings and clinical recommendations.

Identify goals and objectives for treatment interventions, ensuring they are measurable, achievable, and aligned with clients’ needs and preferences.

Serve as a point of contact for clients and their families, providing on-going support, guidance, and advocacy.

Monitor clients' progress towards treatment goals, regularly reassessing needs and adjusting plans as necessary.

Provide immediate telephonic and in person crisis intervention by participating in the IHBT team on-call rotation.

Provide direct support to clients and families during times of acute distress or psychiatric emergencies.

Collaborate with emergency response teams, including crisis hotlines, mobile crisis units, and psychiatric emergency services, to ensure timely and appropriate interventions.

Facilitate individual and family meetings focused on teaching coping skills, emotional regulation techniques, and problem-solving strategies.

Provide psychoeducation to clients and their families on topics related to mental health, illness management, medication adherence.

Assist client and family in accessing community resources.

Work collaboratively with other professionals involved in the client's care. Consults and collaborates with internal and external resources on behalf of the client and family.

Attend agency and departmental meetings.

Complete in a timely manner all required documentation and statistical reporting data including, but not limited to: progress notes, Individual Treatment Plans, discharge summaries, outcomes measures and other reports as required.

Attend supervision, IHBT Team Meetings, agency and departmental meetings as required.

Deliver interventions based on evidence-based practice models.

Ensure service delivery is within the agency and IHBT fidelity requirements by having awareness and knowledge of IHBT fidelity standards and ensuring adherence to these requirements.

Attend agency and IHBT training as required or necessary to maintain compliance with professional licensure and IHBT fidelity expectations.
Qualifications
Education, experience and Skills
Requires at least one year of paid experience working with youth with significant mental health concerns and their families.
Salary : $45,000 - $47,840
