DEPARTMENT: Cameron Family Medicine
JOB TITLE: Nurse Practitioner
SHIFT: Full-time, 80 hours bi-weekly
Summary/Objective
Provides medical direction and treatment of patients within the practitioner’s scope of practice and in collaboration with the supervising physician.
QUALIFICATIONS
- Experience: Minimum of 5 years' experience in treating both acute and chronic conditions.
- Skills/Competencies: Strong communication and critical thinking skills. Ability to multitask and problem solve.
- Education Required: Graduate of an accredited Advanced Practice Provider program
JOB SUMMARY
Provides Medical direction and treatment of patients within the practitioner’s scope of practice and in collaboration with the supervising physician.
ESSENTIAL JOB FUNCTIONS AND PERFORMANCE STANDARDS
- Exemplifies good customer service.
- Collaborates with Multidisciplinary team in office and hospital to assure continuity of care.
- Works to further develop partnerships with the employer hosting the clinic location.
- Collaborates with Multidisciplinary team in office, hospital, and employer clinics to assure continuity of care.
- Conducts a comprehensive assessment of patients, interprets medical histories, performs physical examinations, identifies problems, and formulates a plan of treatment per standard of care.
- Assess normal and abnormal findings from the patient history, physical examination, laboratory test, and other diagnostic findings.
- Documents patient care services in the patient’s EMR, prescribes medications, and discharge instructions in a timely manner.
- Works closely with collaborating physician in treating or referring patients with significant abnormal findings.
- Participates in process improvement.
- Carries out all duties in a manner consistent with Cameron’s guiding principles and values and according to Cameron’s policies and procedures.
- Adheres to the Medical Staff Bylaws, Rules & Regulations, as appropriate.
Experience
- Two years’ experience in designated clinic preferred
Requirements
Education
- Graduate of an accredited Nurse Practitioner program
Licenses Requirements
- Current License in the State of Indiana
- Certified/boarded, if applicable, in their specialty or willing to obtain within six months of hire
- Current DEA certificate without restriction
- Must be able to meet Cameron Hospital’s credentialing requirements.
- Complete a CAQH and update annually as required.
Certifications
- BLS or ability to obtain within one month of hire
