JOB TITLE: Physical Therapist
SHIFT: PRN, As Needed.

JOB SUMMARY
Provides Physical Therapy services following the Indian State Practice Act. Conducts patient evaluations, treatment, and regular progress reports for medically referred outpatients and hospitalized patients.
Certifications/License Requirements
- Minimum Bachelor's Degree graduate of an accredited Physical Therapy program.
ESSENTIAL JOB FUNCTIONS AND PERFORMANCE STANDARDS
- Evaluation and treatment of all patients referred for Physical Therapy, which may include inpatients and outpatients.
- Demonstrates proficient use of hospital required computer programs.
- Takes responsibility for patient’s insurance authorization to ensure financial reimbursement.
Skills / Competencies
- Basic Life Support certified within orientation period according to hospital policy.
License
- Currently licensed as a Physical Therapist in the State of Indiana, or temporary permit.
- Must be eligible for National Registration examination of the A.P.T.A. and pass after a maximum of 2 attempts within 1 year of hire.
Supervisor Responsibilities:
- Supervises rehabilitation aide and Physical Therapist’s Assistants without conducting performance evaluations.
- May supervise PT or PTA students including conducting performance evaluations following 1 year of work experience.
