GENERAL DESCRIPTION OF DUTIES AND RESPONSIBILITIES:
The Crisis Outreach Worker provides compassionate, trauma-informed support to individuals experiencing emotional or personal crisis. This role is working alongside a mental health officer to provide community crisis intervention, outreach, and coordination of appropriate follow-up care. Key responsibilities include conduct comprehensive screening and risk assessments for suicidal behavior, mental illness, substance use, and other vulnerabilities in a community setting Conduct home visits to monitor and report on high needs individuals’ physical, mental, and emotional well-being to the treatment team. Provide solution-focused, strength-based, person-centered interventions to de-escalate crisis. Identify and connect individuals to additional clinical services and support networks. Perform or consult on follow-up contacts with individuals and families. Develop and implement patient treatment plans and comprehensive discharge plans, including relapse prevention strategies. May drive a company vehicle to transport clients to inpatient hospitals or scheduled activities. Expertise in crisis response using evidence-based best practices. Ability to multitask, handle on-site crisis, and adapt to changing needs with compassionate care and attention to detail.
MINIMUM JOB QUALIFICATIONS:
Applicants must meet the following minimum qualifications to be eligible for the position:
- Bachelor’s degree from an accredited college or university, with a major in (or the equivalent to a major), in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention, or a registered nurse or 24 hours in coursework above with a BA in other field of study.
- Must meet TAC Qualified Mental Health Professional (QMHP) qualifications.
- Six (6) months of full-time experience in behavioral health case management.
- Demonstrate ability to read, write, and speak English and Spanish.
PREFERRED JOB QUALIFICATIONS:
- Graduate degree in counseling or social work.
- Prior experience at a behavioral health or community mental health center setting.
- Prior knowledge of the principles, methods, techniques and practices of social work and chemical dependency, the functioning of mental health hospitals and/or out-patient mental health clinics and schools.
- Prior knowledge and experience with trauma-informed clinical interventions for mental and emotional wellbeing.
- Knowledge of applicable mental health laws with regard to mental health warrants, emergency detentions, and out-patient commitments.
- Live within the designated response area.
KNOWLEDGE, SKILLS, AND ABILITIES:
- Ability to work independently, to organize work efficiently, and to communicate effectively both verbally and in writing.
- Knowledge of individual needs/characteristics associated with mental illness.
- Ability to develop productive relationships with individuals and family members that focus on individual specific outcomes.
- Extensive skill in individual needs assessment and the ability to respond to the changing needs of the individual and the ability to work in a crisis.
- Ability to work cooperatively and effectively with other professionals, individuals and groups, exercise tact, initiative, and good judgment when dealing with people, prepare reports and recommendations for administrative use.
- Knowledge of local community resources, how to access those services and use them effectively.
- Ability to advocate effectively for an individual in the target population.
- Knowledge of the principles, methods, techniques and practices of social work and chemical dependency, the functioning of mental health hospitals and/or out-patient mental health clinics and schools.
- Ability to relate to clients with a variety of age, sex, economic, educational, and professional background.
- Ability to apply and integrate theory, knowledge, values, and practices of social work and/or counseling profession.
- Ability to engage in problem solving processes.
- Ability to understand, articulate and apply the dynamics of chemical abuse and dependency.
- Demonstrate skills in the use of personal computers to record client data, written documentation of client assessments and interventions, correspondence, or reports.
- Ability to interpret and assess data.
- Working knowledge of the DSM-IV.
