GENERAL DESCRIPTION OF DUTIES AND RESPONSIBILITIES:
Provides moderately complex case management work in the field of aging and senior care services. Work involves visiting nursing facilities to complete thePreadmission Screening Resident Review (PASRR) assessment required by the Department of Aging and Disability (DADS). The person in this position will beresponsible for the completion of PASRR evaluations for people admitting to or residing in nursing facilities who are identified as potentially having mentalillness or an intellectual and/or developmental disability. These evaluations require face to face interviews with individuals, their family members, nursingfacility staff and others who know the individual, as well as research of existing medical records, for comprehensive discovery of pertinent information to accurately complete the evaluations. Will carry a caseload of nursing home residents that are eligible for services through Camino Real Community Servicesas determined by the PASRR assessment and by the choice of the resident. Will identify and document the services needs of individuals by participating in interdisciplinary team meetings, developing a recovery plan, coordinate, and conduct treatment team staffing's, and coordinate the delivery of necessary services within required service delivery time frames. You must be able to work independently. You must responsibly complete documentation with strict time frames. Must be familiar with and/or will be trained in trauma-informed care. This includes understanding, recognizing, and responding to the effects of all types of traumas on those you come in contact with through day-to-day routines. This position requires you to be detail oriented, possessing computer skills and a good working knowledge of excel and other Microsoft products to data enter assessments and/or prepare required data reports. Must be able to multi-task in astressful work environment. Ex1ensive travel in the service area is required to drive to nursing facilities within the service area. May include some weekend work or late hours. Reports to Crisis Director.

MINIMUM JOB QUALIFICATIONS:
- Bachelor’s degree from an accredited college or university with a major in social, behavioral health or human services field.
- Three (3) years of full-time experience in human services case management and must meet QMHP requirements.
- Must possess proficient computer skills.
PREFERRED JOB QUALIFICATIONS:
- Bi-lingual in English and Spanish.
- Experience working with individuals residing in a nursing facility.
- Prior work experience as a Service Coordinator or Case Manager working with people with intellectual or development disabilities.
KNOWLEDGE, SKILLS, AND ABILITIES:
- Knowledge of community resources
- Must possess excellent verbal and written skills.
- Ability to coordinate, plan and develop services as part of a team and individually.
- Ability to develop productive relationships with individuals and their family members that focus on the individual’s specific goals and dreams.
- Ability to advocate effectively for the best interest of the individuals served.
- Ability to work cooperatively and effectively with other professionals, team members and other agencies.
- Ability to work independently, organize work efficiently and communicate effectively.
- Ability to meet stringent timeframes for submission of paperwork.
- Ability to exercise tact, initiative, and good judgment in different situations.
- Ability to complete documentation of contacts for billing purposes within 48 hours of the contact.
- Must be able to complete all required contacts for assigned consumers monthly.
- Ability to work under conditions requiring physical stamina and ability to work in a stressful environment.
