About the Role – Cardiovascular Patient Advocate Nurse
The Cardiovascular Patient Advocate Nurse plays a vital role in supporting patients throughout their cardiovascular care journey. This position serves as a liaison between patients, families, physicians, and the care team, ensuring that patients receive comprehensive education, care coordination, and advocacy during their treatment and recovery.
What You’ll Do
- Serve as the primary point of contact for cardiovascular patients, guiding them through the care continuum.
- Provide education on cardiovascular conditions, treatment plans, medications, and lifestyle modifications.
- Work closely with physicians, nurses, and multidisciplinary teams to coordinate and optimize patient care.
- Advocate for patient needs, addressing concerns and providing emotional support.
- Assist with discharge planning, follow-up care, and connecting patients to community resources.
- Monitor patient progress and adherence to treatment plans, facilitating interventions when needed.
- Participate in quality improvement initiatives to enhance patient outcomes.
Requirements
- Graduate from an accredited School of Nursing.
- Current valid Georgia nursing license or eNLC nursing license.
- Minimum of one (1) year of cardiovascular nursing experience required.
- Strong communication, advocacy, and patient education skills.
- Ability to collaborate with interdisciplinary teams and provide compassionate, patient-centered care.
